Navigator Program

This program is unique to Trinity Senior Services’ residents and community members. We want to ensure they have a positive experience moving throughout the spectrum of care as healthcare needs change or fluctuate.

Our Vision

The Navigator Program provides individualized support, guidance and advocacy for older adults and their families during periods of transitions, with a special emphasis on the needs of those facing changes in their home environment or in one’s physical or cognitive functioning. The goal of this program is to foster better communication, connection, and support in times of need.

Who We Serve

Prospective Residents

If you are on the waitlist for Milwaukee Catholic Home or Trinity Woods as a prospective resident, you will have priority access to our Rehab Unit and access to our Navigator Program. The Navigator Program helps residents and their families have a smooth transition from the hospital, back to their home or entering the levels of the continuum. This program offers personalized guidance, one on one support, and medical information assistance so that each resident can focus on healing and deciding next housing steps.

Assisted Living

Community Partners

If you are a resident at one of our partnering communities or live in your own home and use our Care Partner Program, you will have priority access to our Rehab Unit and access to our Navigator Program. The Navigator Program provides residents and their families smooth transitions from the hospital to the Milwaukee Catholic Home and back to their home community. This program offers personalized guidance, one on one support, and medical information assistance so that each resident can focus on healing. These partnerships allow for residents to get the best care possible spanning over multiple communities.

Milwaukee Catholic Home Residents

As a Milwaukee Catholic Home Resident, you will have priority access to our Rehab Unit and access to our Navigator Program. The Navigator Program provides residents and their families smooth transitions from the hospital, or between the levels of the continuum. This program offers personalized guidance, one on one support, and medical information assistance so that each resident can focus on healing.

Trinity Woods Residents

As a Trinity Woods resident, you will have priority access to the Milwaukee Catholic Home Rehab Unit and access to our Navigator Program. The Navigator Program provides residents and their families smooth transitions from the hospital to the Milwaukee Catholic Home and/or back to Trinity Woods. This program offers personalized guidance, one on one support, and medical information assistance so that each resident can focus on healing. Trinity Wood’s connection to the Milwaukee Catholic Home allows for residents to get the best care possible spanning over multiple communities.
