Assisted Living
We believe you deserve a personalized plan of care that supports your health and independence. While other assisted living facilities may take a one-size-fits-all approach, you will find we offer a variety of assisted living options, whether you need limited supportive services in your own apartment or more structured care. We have assisted living options at both our Milwaukee Catholic Home and Trinity Woods communities. Milwaukee Catholic Home also offers assisted living respite as an option for older adult seeking a short-term stay.
Assisted Living Memory Care
You or your loved one with memory challenges brought on by Alzheimer’s disease or dementia deserve a caring, homelike environment that supports health and independence. Clare Garden Suites at Milwaukee Catholic Home is made up of two memory care-specific neighborhoods: Garden Suites and Clare Suites.
Skilled Nursing
We believe residents with long-term and memory care needs deserve a rich and robust living situation, inspiring us to provide a quality care environment surrounded by the people and experiences that make life meaningful. Working with your strengths, our attentive and dedicated staff is committed to help you live your life to its fullest. We have skilled nursing living options available at Milwaukee Catholic Home.
Transitions Rehabilitation
A time of healing should be personal, private and purposeful. Thanks to our innovative rehabilitation approach, your experience can be a smooth transition back to daily life. Whether preceded by an injury, illness or surgery, transitions rehabilitation care offers a clinically sound and seamless transition home.
Our exceptional rehabilitation team works with your doctor to develop a care plan that meets your goals and needs. Along with the expert medical care of our doctors and nurses, you will have support from physical, occupational, speech and recreational therapists, dietitians and social workers. We have transitions rehabilitation options available at Milwaukee Catholic Home.
