What is a Life Plan Community?

A life plan community is where planning and living merge. Milwaukee Catholic Home offers a continuum of care that includes independent living, assisted living, memory care, rehabilitation and long-term skilled nursing. Sometimes known as a continuing care retirement community, a life plan community gives residents and their families the security and peace of mind of knowing that our individualized care adapts if your needs change.

Our whole person wellness philosophy is of particular benefit to our residents with memory loss and also addresses the needs of residents who experience depression, loneliness, isolation and a variety of chronic health concerns. It improves the overall quality of life for our residents in the context of an interdependent and supportive community.
