Assisted Living

We believe you deserve a personalized plan of care that supports your health and independence. While other assisted living facilities may take a one-size-fits-all approach, you will find Milwaukee Catholic Home offers a variety of assisted living options, whether you need limited supportive services in your own apartment or more structured care. We partner with you to create a service plan that fits your lifestyle and needs, including weekly health monitoring and around-the-clock staff presence.

Assisted Living Respite is an option for older adults seeking a short-term stay in an assisted living home environment of comfort, freedom and individualized support.

Assisted Living Memory Care

We believe you or your loved one with memory challenges brought on by Alzheimer’s disease or dementia deserve a caring, homelike environment that supports health and independence. Our Clare Garden Suites is made up of two memory care-specific neighborhoods: Garden Suites and Clare Suites. We offer four levels of assisted living memory care, enabling you to live in comfort, safety and security.

Within Clare Garden Suites, service plans are coordinated by a dedicated and highly trained team of professionals working together to meet your needs and goals while supporting overall wellness. Regardless of the severity of your memory loss, we offer individualized plans to ensure the best care. Clare Garden Suites offers a variety of apartment types across assisted living and for those with more skilled needs, skilled nursing.

We know cognitive and physical decline can be the most difficult challenges faced by older adults. Our exceptional memory care services, Everyday Amazing, is evidence-based and research-driven.

Skilled Nursing

We believe residents with long-term and memory care needs deserve a rich and robust living situation, inspiring us to provide a quality care environment surrounded by the people and experiences that make life meaningful.

Here you’ll experience an active, warm community while still having a private room to make your own. Working with your strengths, our attentive and dedicated staff is committed to help you live your life to its fullest.

Transitions Rehabilitation

A time of healing should be personal, private and purposeful. Thanks to our innovative rehabilitation approach, quality care team and respect for your healing journey, our rehabilitation experience is a smooth transition back to daily life. Whether preceded by an injury, illness or surgery, transitions rehabilitation care offers a clinically sound and seamless transition home.

Our exceptional rehabilitation team works with your doctor to develop a care plan that meets your goals and needs. Along with the expert medical care of our doctors and nurses, you will have support from physical, occupational, speech and recreational therapists, dietitians and social workers. Private rooms give you the quiet and space to support the healing process.

Along with a recent deficiency-free state survey and a history of five-star ratings from the Centers for Medicare and Medicaid Services, we are proud to have our Transitions Rehabilitation unit designated as “Best Short-Term Rehabilitation” by U.S. News and World Report.
